APPLICATION FOR EDUCATIONAL LICENSE

Please identify the license requested by checking one of the two boxes below. By checking the box, you certify that you satisfy the user qualifications for an educational license as set forth below.

Section 1: User Qualifications

Individuals:

[  ]  I certify that I meet the requirements for an educational license (as checked below) at the Institution stated. I am purchasing a license to the Vox Proxy software (the "Software") for my own personal use for educational purposes only and not for commercial use.

[  ]  I am a full or part-time student currently enrolled at an accredited public or private university or college, including community, junior or vocational college that grants degrees. I have attached a copy of my current dated ID or other valid proof as stated in the Vox Proxy Educational Pricing Qualifications.

[  ]  I am a full or part-time faculty or staff member currently employed by an accredited preschool, K-12, or higher educational institution, as defined in the Vox Proxy Education Pricing Qualifications. I have attached a copy of my current dated ID or other valid proof as stated in the Vox Proxy Educational Pricing Qualifications.

[  ]  I am a registered K-12 student. I have attached a copy of my current dated ID or other valid proof as stated in the Vox Proxy Educational Pricing Qualifications.

Institutions:

I certify that this institution meets the requirements for an educational license as stated in the Vox Proxy Educational Pricing Qualifications. This institution is purchasing a license to the Software for internal educational, administrative or instruction purposes only.

Section 2: Terms and Conditions of Use

The undersigned acknowledges that the user of the Software is bound by the terms and conditions of the SOFTWARE LICENSE AGREEMENT FOR Vox Proxy software set forth in the click license that appears upon installation of the Software. If the undersigned has applied for a site license, the undersigned acknowledges that the Software may only be accessed and used at the Designated Site or otherwise in accordance with the terms and conditions of the SOFTWARE LICENSE AGREEMENT and that access is limited to the number of licensed users as specified above. The undersigned further acknowledges that the Software is licensed for educational purposes only and that any commercial use of the Software will immediately terminate the undersigned's right to use the Software.

Section 3: Entire Agreement

The undersigned acknowledges that the undersigned had read this agreement, understands it and agrees to be bound by its terms and conditions. The undersigned further agrees that this agreement, together with the SOFTWARE LICENSE AGREEMENT FOR Vox Proxy (the "click license"), is the complete and exclusive statement of the agreement between the parties which supersedes any proposal or prior agreement, oral or written, and any other communications between the parties relating to the subject matter of this agreement. All terms and conditions of the SOFTWARE LICENSE AGREEMENT FOR Vox Proxy remain unchanged and are applicable to this agreement.
